How the Reward Program Works

If you’re willing to take a few minutes to complete a life insurance application and view your personalized offer, we will thank you for your time by rewarding you with a $25 Digital Gift Card from Amazon — whether you choose to purchase coverage or not.

Here’s how to qualify:

1. Complete the application using the provided QR code or personal landing page link.
   (Applications submitted through Ethos.com or any other website do not qualify.)
2. When you reach the final offer page (the last screen of the application), take a screenshot.
3. Text the screenshot to 602-960-9783, along with your name.
4. One of our agents will call to confirm your identity and verify your email address
5. Once verified, we will email your $25 Amazon gift card. No purchase required.

IMPORTANT: This reward is simply a thank-you for your time — not an incentive to purchase or accept coverage.

Use of the Life Matters Landing Page is Required

To be eligible for the reward, the application must be:

• Started and completed using the provided QR code 

• Submitted through the Life Matters Ethos agent landing page

We will verify your application submission through our back office account, if it is not there, we will require a new submission before issuing the reward, even if a screenshot is provided. Applications completed directly on Ethos.com, through another agent, or through any unrelated link are not eligible. This ensures the program remains fair and tied to the link provided.

Why a Verification Call Is Required

Programs like this can attract bots, duplicate attempts, or fraudulent submissions. To keep things fair and honest, we require a brief verification call to:

• Confirm your identity

• Confirm your email address so we can deliver the Amazon Gift card reward

• Verify that the application was completed through my landing page and is legitimate

Eligibility Requirements

To participate, you must:

• Be age 18+

• Be a U.S. resident

• Only one reward request per person and per household

• Complete the application yourself, using the provided QR code or landing page link from the card

• Reach the final offer page of the application

• Provide accurate contact information for verification

A valid phone number and identity confirmation is required before issuing the reward.

Reward Delivery & Time Limits

• Rewards are typically sent within 3 business days after successful verification

• Delivery is provided as a digital Amazon gift card

• If we are unable to reach you or complete verification within 7 days of receiving your screenshot submission, the reward becomes void and will not be issued

• After expiration, you may re-apply for a new application using the same link and landing page.

The following will not be eligible:

• Applicants under the age of 18

• Applicants outside of the US

• Applications not completed through my QR code or landing page that do not appear in my agent back-office records

• Incomplete or abandoned applications prior to the final offer page

• Quote screens or pages that are not the final offer page

• Screenshots from someone else’s application

• Duplicate or repeated submissions

• Fake names, unverifiable contact information, or identity mismatch

• Automated, bot-generated, or suspicious activity

• Participants who refuse or fail identity/email verification

Fraudulent activity will be blocked from future participation.

Program Terms

• The reward is provided at my discretion and may be modified, paused, or discontinued at any time

• Participation does not require purchasing a policy

• Receiving or declining the reward does not affect underwriting, pricing, or eligibility decisions

• Amazon and Ethos are not sponsors of this promotion, are not affiliated with this program, and do not endorse or participate in it in any way

• All Amazon and Ethos trademarks are the property of their respective owners

• By submitting a request, you acknowledge and agree to these terms
